Dave Cundiff, MD, MPH is an experienced leader in the field of Substance Use Disorder treatment. He works with patients suffering from Substance Use Disorder to evaluate their medication needs and prescribe treatments accordingly. In addition, he regularly participates in all-staff debriefing sessions involving peers, nurses, and other prescribers. He also reviews and advises on policies, procedures, and techniques for treating substance use disorder.

Dr. Gladish is a Board Certified Adult General Psychiatrist. She is originally from New Jersey but has called Portland home for the last 12 years. Prior to medical school, she completed her Master’s in Public Health in health education and promotion at Tulane University’s School of Public Health and Tropical Medicine in New Orleans. She attended medical school at the Flinders University of South Australia’s School of Medicine in Adelaide, South Australia. She undertook her training in psychiatry at Yale University, The Royal Australian and New Zealand College of Psychiatrists, the University Hospital, Zurich Switzerland, and Oregon Health and Science University.

She has worked in numerous psychiatric settings including emergency psychiatry, adult inpatient and outpatient psychiatry, and geriatric inpatient and outpatient psychiatry. In the Portland area, she has worked at Providence, Tuality, Cascadia, and has been on faculty as an Assistant Professor at OHSU in a joint position between OHSU and the Unity Center for Behavioral Health.

She has an interest in biological approaches to treating depression and other mental illnesses. She has pursued additional training in Transcranial Magnetic Stimulation at the Berenson-Allen Center for Noninvasive Brain Stimulation, at Beth Israel Deaconess Medical Center/Harvard Medical School, Boston, Massachusetts.

Outside of work she enjoys surfing, hiking, road cycling, gardening and spending time with her husband and dogs.

Carly was drawn to becoming a therapist because she is passionate about helping people understand themselves and their experiences in a way that promotes healing and empowerment. Driven by the belief that everyone has the potential to rewrite their story, especially when it comes to navigating intersecting identities and societal challenges, Carly is deeply committed to creating spaces that affirm people’s lived experiences and help them connect with a more authentic and meaningful version of themselves.

Carly received her Master of Social Work degree in 2018 from the University of Southern California and became a Licensed Clinical Social Worker in 2021. She has furthered her education by pursuing advanced training in clinical supervision, EMDR, narrative therapy, internal family systems therapy, dialectical behavioral therapy, SMART recovery groups, and Seeking Safety groups, among others. Carly has experience providing individual, family, and group therapy across a variety of settings, including residential facilities, schools, partial hospitalization and intensive outpatient programs, foster care group homes, and juvenile detention facilities.

Outside of work, Carly spends much of her time taking advantage of Portland’s beautiful parks, hikes, cafes, and playgrounds with her husband and daughter. Carly is a lifelong equestrian and hopes to someday pursue training to become a certified Equine Assisted Psychotherapy practitioner.

Bruce D. LeBlanc, PhD, EdD, MAPAC, MA

Bruce facilitates a sexual health group for the men’s treatment program, as well as a spirituality group for individuals in residential treatment. Bruce draws upon his doctorates in psychology, education, and his master’s in addiction studies, as well as spiritual psychology, when helping clients.

“I believe that one’s sexuality is an integral part of who they are, and having a healthy sense of one’s sexuality is a part of one’s living a healthier, sober life. I also believe that embracing one’s soul life from a psychological, spiritual, and/or religious perspective can provide an additional foundation to draw upon as one journeys through recovery toward living a sober life.

I have been in long-term recovery since 2019, having overcome substance use and alcoholism. My own journey through recovery has not only transformed my life but has also inspired me to dedicate myself to helping others achieve and maintain sobriety. I am an active member of the recovery community, and I hold numerous long-term service commitments that allow me to give back and support others on their path to healing.

As a case manager, my role involves working closely with individuals in recovery to provide the guidance, resources, and support they need to achieve long-term success in their sobriety. I am deeply passionate about my work, and it is my personal experience and commitment to recovery that fuels my desire to help others navigate the challenges of early and ongoing recovery.

Pashia is a Billing Specialist at Crestview Recovery. She has over 10 years of administration experience and background in medical billing. In her free time Pashia enjoys traveling and spending time with her family.
